All registrations, whether, phone, fax, mail or walk-in, require a registration form and signed waiver with payment before you are enrolled in a program or trip. We are unable to hold your place in a program without proper registration and payment. A fee of $10.00 will be assessed on all checks that are returned from the bank due to insufficient funds. Phone-In Registration You may phone in your registration by calling 815-786-8044. Visa, MasterCard or Discover are accepted as payment when you choose to register by phone. A receipt will be mailed to you to confirm your payment and registration. Walk-In Registration You may choose to register in person at the Park District office, located in the David A. Francis Center, Knights Park, 1001 N. Latham Street. The office is open from 9:00 a.m. to 8:00 p.m. Monday through Thursday, 9:00 a.m. to 2:00 p.m. on Friday and 9:00 a.m. to 12:00 p.m. on Saturday. Mailed or Faxed Registration Registrations received by mail or fax will be processed as soon as they are received. If a program is already full, you will be notified as soon as possible. Our fax number is 815-786-6690. Payment MUST be included with your registration. Please make checks payable to the Sandwich Park District. If you are paying with a credit card, please include your credit card information on the registration form. (Download a registration form) On-Line Registration Within the next few months we hope to make on-line registrations available to Park District patrons. Fees - Resident versus Non-Resident Sandwich Park District residency is determined as residing with the Sandwich Park District taxing boundaries. The Sandwich Park District was established in 1966 to include only the City Limits of Sandwich. Anyone living outside these boundaries such as Lake Holiday, Wildwood, Hidden Oaks and rural route addresses are termed non-residents and will pay a slightly higher fee for programs. Park District residency is based on the address where the participant lives and not their business address of other family members living in-district. If taxes are paid to the Sandwich Park District on the home in which you reside, you are a Sandwich Park District resident and are entitled to a resident fee. A non-resident card is available for purchase to those living outside the Sandwich Park District taxing boundaries and offers your family "resident" rates for one year from date of purchase. Non-resident cards may be purchased at the David A. Francis Center in Knights Park. Refunds If the Sandwich Park District feels the need to cancel a program, a full refund will be given. All other trips and programs require 48 hours notice in order to receive a refund. NO refund will be given on trips that include any type of reserved or pre-purchased ticket. It is the participant's responsibility to find someone to take their place should they find the need to cancel.
